If you live in our catchment area and wish to join our practice, please attend the surgery to collect the registration paperwork.
From 14 November 2017 you will need to bring a form of ID preferably with a photo and if possible a proof of address at the time of registration together with your medical card.
This will also enable us to register you for our online service.
Once you are registered you will be offered an appointment with our Nursing Team for a new patient medical check. If you are on repeat medication, you will need to make an appointment with one of our doctors to review your medication before you can request anymore. Please ensure that you have enough supply from your previous surgery until your appointment for your medication review.

Online Service
Once we have signed you up to the online service we will provide you with your username and password.
You will be able to book your appointments, request medication and view part of your medical records from the online service.
The Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
Our practice policy allows patients to choose whichever doctor they wish to see. For continuity of care please try to see the same doctor for a particular problem. Your rights and responsibilities are set out in our Patient Charter. We strongly support the NHS policy on zero tolerance, Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.
